Meet Sherryl Eisma, FNP-C
Sherryl Eisma, FNP-C, is a board-certified family nurse practitioner dedicated to helping individuals achieve optimal health through an integrative approach. She holds a Bachelor of Science in Nursing and a Master of Science in Nursing from Texas Woman's University.
After completing her graduate studies in 2016, Sherryl has offered primary health care for over 8 years to thousands of individuals. With a passion for whole-body wellness, she specializes in medical weight management, hormone optimization, chronic disease management, and primary care for all ages. You'll have a primary health care provider who knows you personally, has your detailed medical history and will make sure your healthcare is custom designed to fit your personal needs.
- Accessible
- Scheduling an appointment with your Primary Care Provider should be easy. At Inspire Personal Health we offer extended, same-day or next day appointments, as well as access to your provider by phone, email, or text. If there is an urgent matter during non-business hours, we encourage you to contact us. When we say we're here for you, we mean it.
- Affordable
- We offer a low-cost monthly subscription membership fee without billing insurance. That means less hassles, no deductibles or copays, and heavily discounted labs. (All services that can be done in person will be covered under your membership fee.)

What does a membership include?
- Unlimited in-home/virtual primary care visits during office hours.
- After hour or weekend office visits upon request.
- Unlimited health care access by phone, email, and text.
- No charge for tests done in-home such as rapid strep, rapid flu/Covid, urinalysis, urine pregnancy tests, fingerstick blood glucose.
- In-Home visits are available for patients that live within 10 miles of Wylie, TX
- Virtual visits are offered to anyone in the wonderful state of Texas!

Children 18 and under are only $25 per month with adult membership
Maximum family monthly cost of $225 per month
Virtual Membership $50 per month for adults anywhere in Texas

5% discount for 6 month subscription
10% discount offered for annual subscription
One time registration fee of $50 per member or $100 per family
